Registration Form Academic Year 2017-18
One-on-one ACT, SAT, math or science tutoring, or academic coaching
When selecting your availability on the registration form, please select every time your child could come in for tutoring. You can indicate your preferences, too. If we don't have any times available during the availability you provide us with, your registration will be canceled and we will refund your deposit. To avoid delays, PLEASE include all available times. We really do try to honor your preferences!
Instructions:
1.) Fill out the form below. PLEASE provide complete availability.
2.) You will be prompted to pay the non-refundable deposit of $150 after you submit your form. If we do not receive this deposit within 12 hours of the time we receive your registration form, we will delete your registration because we assume you've changed your mind. If you would rather pay your deposit by check, this is fine. You will need to check the appropriate box at the end of the registration form.
3.) If we are unable to work with your child due to scheduling conflicts or any other reason on our end, we will refund your deposit immediately. Please do not submit a registration form and wait to hear from us regarding scheduling before submitting a deposit -- we will not schedule your child without a deposit.
4.) After we receive both your registration form and your deposit, we will email you at the address you give us at the top of the form. We will schedule your student to begin tutoring at the earliest possible time. Academic tutoring and coaching students can expect to start tutoring within a few days. Test prep students will begin tutoring according to their time table.

Please note that this deposit is not refundable if you decide not to do tutoring. Your deposit, however, will be applied toward tutoring if you decide to register again within 180 days. Your deposit will be refunded if we cannot fit your child onto our schedule. After you submit the form and see the confirmation message above, go to the Payments page to make your deposit.
